Office Hours:
The office is open from 8:00am-5:00pm on Monday, Wednesday, and Friday.
The office is open from 8:00am-6:00pm on Tuesday and Thursday.

After Hours Emergency:
If you require emergency assistance from a doctor after office hours, please dial the office directly at 561-689-5500, and an operator will assist you. Please be sure to leave your name, phone number, and the nature of your emergency with the operator. If you do not hear back from the doctor on call, or if you are having a true medical emergency, please proceed to the nearest emergency room or urgent care center for help.

Financing:

Release of Records:
Should you require copies of your medical records, you will need to complete a records release form. Please note the following in accordance with Florida Statute: For copies of chart pages, a minimum of 10 (ten) working days and not more than 30 (thirty) is required to process your request. These copies will be billed to you at a rate of $1.00 per page for the first 25 pages and $0.25 per page after that, payable prior to release of your copies.
